Care coordination in advanced illness

Malene Davis describes how Capital Caring has evolved in response to the growing need for increased awareness of and access to hospice and palliative care services in the Washington DC area. Capital Caring seeks to help families and their loved ones make the most of every moment together by offering coordinated care, comfort, and dignity to those living with serious illness. Pain and symptom management as well as emotional and spiritual support are structured in the plan of care. By integrating hospice and hospital care and expanding services to include individuals with long-term needs, their care coordination models expand conventional ideas of hospice to increase the number of patients who can benefit. At the same time the cost burden of hospital stays and readmission is lowered. Capital Caring is interested in partnering with physicians who are looking to refer patients, social workers who desire continuing education, and providers in the region who are interested in collaborating.
